As part of onboarding you to write or place business through Hagerty, Hagerty may obtain one or more consumer report(s) about you pursuant to the Fair Credit Reporting Act (FCRA) as part of the license verification process. These reports may include, but are not limited to, reports from the National Insurance Producer Registry (NIPR) and the Producer Database (PDB) which include your license status, lines of authority, appointments, and any regulatory or disciplinary actions reflected in those records. This consumer report may also include information as to your character, general reputation, personal characteristics, and mode of living. This information may involve information obtained from personal interviews with others about you, such as your present and former employers or references supplied by you.

Any such consumer report will be obtained for the purpose of verifying your license status, qualifications, and determining your access and permissions within Hagerty's producer systems.

Authorization: I have read and understand this FCRA Disclosure and Authorization. I authorize Hagerty to obtain consumer report(s) about me for the purposes described above.
